+- `core/chunks/groups.ts`: `ToolEntry` replaces `ToolBatchEntry`; every call becomes an independent
+  `tool` render group, paired by `toolCallId` (including legacy chunks without `stepId`). Duplicate
+  calls/results are deduped first-occurrence-wins; results whose calls were unloaded remain visible
+  as orphan cards. Call/result provisional flags and step/turn provenance are retained.
